    Evidence-based decision making is a common practice in health care management. However, the organizational changes and the growing need for health care service quality, encourages to search for more sensitive methods for quality flaws data identification. Patient journey as patient experience’s analyses could be considered as such new approach providing an appropriate information about direction for development and rapid decisions. The purpose of this paper is to investigate the patient's experience during their journey and determine the flaws in health care system service quality, giving priority to organizational dynamics. The study is based on focus group qualitative interviews. Patient travel mapping technology was chosen for the study. The study used the most popular breakdown of the patient's journey stages: diagnosis; treatment; rehabilitation; follow out; monitoring in order to find suitable management elements within the standard stages. Discretion of stops draws more attention to the intermediate level of medical specialist chain at the diagnosis stage. The data analyses revealed that dynamical health care quality issues are raised from existing interorganizational barriers. The patient experience could help to find the managerial gaps between health care providers in the system and to “measure” the cooperation level. This measuring occurs when patients try to describe their experiences by linking them to travel stops and try to differentiate their experiences at different stages of the disease. Research reveals an inequality in the quality of service provided by the health care institution. The most significant differences are revealed in the comparison between the content of the service, place, moment, provider. Patient journey application framework based on organisational dynamics (organizational environment, rule and procedural-driven and inter-organizational constrains) have been proposed. Summarising, the patient journey approach can help to figuring out the quality elements needed the instance attention and extract data for decisions making.

    The patient journey (PJ) as a health care quality improvement construct becomes an instrument of management, which aims to increase the effectiveness of health care. Its guiding principle is that the patient experience throughout each stage of interaction with healthcare providers can reveal gaps in service quality and directions for improvement. However, the mapping of PJ raises a number of unresolved issues, such as the issue of discretization of PJ stages, the integrity of PJ in terms of the entire healthcare system, and the unresolved issues of standardization of PJ mapping. The aim of this study is to assess the PJ mapping methodology, adapting it so that the patient's journey reflects the integrity of the entire system, beyond a specific health care institutions frames. Methodology. The study was carried out between July and September 2020. A method of story-telling and semi-structural interview was chosen for patient journey mapping. The study involved doctors from different levels (5 doctors) and patients (5 patients). Reconstructed and generalized PJ has been validated in a selective group in order to screen the critical moments of journey. The study is based on the classical and most popular breakdown of the patient's journey stages: 1) the first symptoms; 2) diagnosis; 3) treatment; 4) rehabilitation; 5) follow out; 6) monitoring and prevention in order to find suitable standardized management elements within the standard stages. Findings and conclusions: The study showed that the patient's journey in Lithuania is characterized by a large number of stops. Furthermore, patients have to go through too many stops at one institutional level until they can proceed further. The PJ is quite clear and transparent in stages of a single health care level but the integration and coordination amongst levels and sectors is limited. Public health/long care nursing/social services as parts of the PJ are separated, the experience of using such services is chaotic. In order to create an integral scheme of the PJ which could be adapted for improving the quality of services on both an institutional level and in terms of the entire healthcare system, it is important to differentiate all possible sublayers of the journey. 3 sublayers of the journey were determined: service dimension, specialization of healthcare institution and patient emotions. It is also necessary to explore the contents of service sublayers, such as prevention, social services, etc. Thus, the stages of the patient journey become multidimensional with multi-actor interface.

    Moterų ir vyrų pajamų atotrūkis Lietuvoje : monografija

    Moterų ir vyų darbo užmokesčio bei pajamų atotrūkio tema yra nagrinėta daugelio autorių pasaulyje ir Lietuvoje. Tačiau monografijoje išryškintos Lietuvoje gana nelauktos tendencijos ir pokyčiai bei pateikti nauji tyrimų rezultatai lyčių lygybės srityje. Remiantis autorių atlikto tyrimo išvadomis, žvelgiant iš rinkos ekonomikos 30 metų laikotarpio retrospektyvos, Lietuvoje lyčių lygybės srityje pasiekti įspūdingi rezultatai. Svarbiausi pasiekimai lyčių lygybės srityje – per 30 metų pasikeitė lyčių lygybės stereotipai, pastebima sparti moterų dalyvavimo darbo rinkoje pažanga, pasikeitė darbdavių požiūris į moterį – darbuotoją: darbdaviai aukštai vertina moterų darbo potencialą, jų išsilavinimą ir profesines savybes. Tačiau, nepaisant pasiektos pažangos, atlikto tyrimo rezultatai leidžia iškelti Lietuvoje naują akcentą ir iššūkį lyčių lygybės srityje – didelę moterų diskriminaciją pagal amžių, ypač priešpensinio ir pensinio amžiaus moterų. Ši problematika nors ir buvo nagrinėjama Lietuvoje, tačiau dažnai suvokiama kaip savaime suprantama, kad pajamos, sulaukus pensinio amžiaus, ir turi mažėti. Autorių pozicija yra kita – vyresnio amžiaus žmonių profesinis potencialas yra didžiulis neišnaudotas šalies ekonomikos resursas ir tikslinga jį efektyviai panaudoti šalies darbo rinkoje. Sparčiai didėjant visuomenės senėjimo ir emigracijos iš Lietuvos rodikliams, turėtų didėti vyresnio amžiaus žmonių paklausa darbo rinkoje. Sukaupta vyresnio amžiaus žmonių darbo patirtis, žinios, profesinis potencialas turėtų būti panaudoti Lietuvos ekonomikoje, ypač sparčiai mažėjant darbo jėgai dėl didelio masto emigracijos iš šalies. Autorių atlikti darbuotojų ir darbdavių sociologiniai tyrimai leido atskleisti gilumines lyčių, ypač vyresnio amžiaus žmonių pajamų skirtumų priežastis. Kaip vieną iš svarbių monografijos privalumų galima pažymėti ir tai, kad darbas turi ne tik teorinę, bet ir praktinę reikšmę darbuotojams ir darbdaviams vertinant moterų ir vyrų darbo užmokesčio skirtumus. Pasiūlytas LOGIB metodas leidžia palyginti, greitai ir paprastai įvertinti moterų ir vyrų padėtį darbo užmokesčio srityje visoje Lietuvoje ir bet kurioje verslo įmonėje ar įstaigoje. Tai – naujas žingsnis lyčių ekonomikos mokslinių tyrimų srityje. Darbas yra inovacinis, turi mokslinę ir praktinę vertę, jis būtų naudingas mokslo bendruomenės atstovams, politikams, ekonomistams, verslininkams, darbdaviams, darbuotojams, studentams, nevyriausybinių organizacijų atstovams, besidomintiems lyčių lygybės klausimais.
